SUMMARY

The director supports the development and implementation of population health and value-based care initiatives using data-driven approaches to enhance patient care across the continuum and improve quality outcomes. The director is responsible for the supervision of population health care coordination staff and value-based care program implementation, including transitional and complex care management programs, remote patient monitoring program, contract performance oversight, ambulatory quality outcomes reporting, and continuous quality improvement efforts. The director is accountable for the oversight of thorough, timely, ongoing/recurring reporting processes and the correlating data sharing and education with organizational stakeholders. The director will lead all aspects of the GA-AIDE Directed Payment Program with leaders across clinical operations, information technology, business, and clinical intelligence and finance. This leader will also partner closely with ambulatory leaders including nursing, physician, and administrative leadership to ensure quality and performance metrics are clearly defined and necessary workflows, interventions, and standard work practices are in place and executed.

QUALIFICATIONS

Bachelor's degree in nursing healthcare administration, Health Policy, Business Administration, or related field such as Occupational Therapy, social work, psychology, or counseling with professional licensure in a clinical field.

Preferred Education: Master's Degree in Nursing or a closely related field. Master's in Social Work or other healthcare related field accepted.

Six (6) years of experience in healthcare with progressive leadership.

Minimum two (2) years of healthcare quality improvement and healthcare data analytics.

Minimum Seven (7) years clinical practice with prior experience in ambulatory settings or population health programs.

Minimum three (3) years in a manager or other leadership role related to Care Management or Care Coordination.

Previous experience in building, leading, growing, and managing population health-oriented care coordination programs required.

Project Management Professional Certification (PMP) - preferred

Certified Professional in Healthcare Quality (CPHQ) - preferred

Certification in case management (CCM) preferred.
